Official Rules for CFE SJC Study Sweepstakes
Public Works Partners (PWP), on behalf of Cities for Financial Empowerment Fund (CFE), is conducting research on Summer Youth Employment Program participants in the summer of 2015. Public Works Partners is administering a sweepstakes to encourage participation in the online survey component of this research.
Summery Youth Employment Program (SYEP) participants that submitted a complete, signed consent form in June, July or August 2015 noting his/her consent to participate in the Summer Jobs Connect Financial Empowerment Study, conducted by PWP AND receive an invitation to complete the Summer Jobs Survey by August 25, 2015 will be automatically entered into the sweepstakes, and therefore considered an eligible entrant. Survey completion is not required to be entered into the sweepstakes. No additional activity is required to be entered into the sweepstakes.
Anyone who receives an invitation to complete the survey will be entered into the sweepstakes, whether or not the invitee participates in the survey. Receiving an invitation to participate does not require participation in the survey.
Anyone invited to participate in the survey that wishes to be withdrawn from the sweepstakes, regardless if that person completes the survey or not, can be withdrawn by notifying Travis Bostick at Public Works Partners at email@example.com.
Prize and Prize Distribution
10 Samsung Galaxy Tab 4 7.0” 8GB (Wi-Fi enabled) will be awarded per the prize distribution parameters described below as part of the Summer Jobs Connect Financial Empowerment Study Survey Sweepstakes. Each tablet is valued at approximately $180 USD. PWP will not provide, nor is responsible for, any accessories, data plans or other items associated with the tablet.
No later than October 1, 2015, PWP staff will conduct a random drawing of all eligible sweepstakes entrants in each of the six cities where sweepstakes are being conducted: New York, NY; Chicago, IL; Miami, FL, Washington, DC; Los Angeles, CA; and San Francisco, CA.
In each of the following cities: New York, NY, Chicago, IL, Washington, DC and Los Angeles, CA: two winners will be selected to win one Samsung Galaxy tablet each.
In each of the following cities: Miami, FL and San Francisco, CA: one winner will be selected to win one Samsung Galaxy tablet.
Odds of winning depend on the number of entrants. PWP may decide to increase the number of winners in a particular city at its own discretion without notifying participants.
PWP will notify winners no later than October 15th in the same manner that invitations to participate in the survey were sent: Sweepstakes entrants that received an email directly from PWP will be notified at that same email address. Sweepstakes entrants that were asked to participate by a representative of the community based organization or city agency that is coordinating their summer employment will receive notification from the community based organization or city agency. All notified winners will be directed to claim their prize at the offices of the community based organization or city agency that is coordinating their employment (exact location will be included at the time of notification). All winners must claim their prize by October 30th, 2015.
Taxes and Liability
All federal, state and other tax liabilities arising from the sweepstakes are the sole responsibility of the winner(s).
PWP is not responsible for lost, late, stolen, incomplete, illegible, inaccurate, undelivered, deleted or misdirected entries or contact information.
PWP reserves the right, in its sole discretion, to modify or terminate this sweepstakes if an act, occurrence or reason arises that PWP believes could corrupt the integrity, administration or fairness of the sweepstakes.
By participating in the sweepstakes, participants agree to release, discharge and hold harmless, PWP and CFE from any liability, claims, losses and damages arising out of or relating to their participation in the sweepstakes or the acceptance, use, misuse or possession of any prize received in the sweepstakes.
